Our Blue Cross benefits include a medical travel benefit for ALTA members.
The Benefit is as described:
- Maximum for Mileage: $0.28 per KM,
- Maximum for Accommodation: $50 per day for 7 days
Step 1 
Before you travel to your out of town medical, you should know that you will require proof of attending your appointment to Blue Cross. In order to do this you should bring a copy of our ALTA Medical Travel Form.
Step 2 
You will need to fill out a Health Claim Form from Blue Cross you can use the mileage calculator below in order to calculate the amount you will claim for reimbursement.
Step 3 
If you have to stay at a hotel you can claim $50 per night in expenses for overnight stay. Include any hotel receipts in your claim.
Step 4 
Put all the documents in an envelope and mail the claim to:

Note: You may want to make a copy of all your documents prior to mailing them. You will not get any documentation back from Blue Cross.
